ADHD and Hidden Hearing Difficulties

Hearing vs Listening: Why the difference matters

Listening, by contrast, is an active, cognitive process. It involves directing attention, holding information in memory, filtering out irrelevant sounds, and interpreting meaning—all of which rely on higher cortical functions, particularly in the prefrontal cortex and auditory association areas.

This distinction matters immensely for people with ADHD. Most adults with ADHD have no issues with the peripheral hearing system, often reported as having “normal hearing” on a hearing test. But they may struggle with listening, especially in environments with multiple competing sounds or when complex verbal information is presented. These listening difficulties stem from the same executive function challenges that affect focus, working memory, and inhibition in other areas of life.

What is the research telling us?

Key research findings include:

  • Increased listening effort: A 2020 study in Frontiers in Psychology found adults with ADHD expend significantly more cognitive effort to follow speech in noise, even when hearing thresholds are normal. This increased effort can lead to mental fatigue and reduced speech comprehension over time.
  • Delayed auditory event-related potentials (ERPs): Multiple neurophysiological studies have shown that individuals with ADHD often exhibit delayed or attenuated ERP responses to auditory stimuli. These are markers of how the brain processes sound, particularly in tasks that require attention. A meta-analysis published in Neuroscience & Biobehavioral Reviews (2018) found consistent differences in the P300 waveform among adults with ADHD, which is linked to attention allocation during auditory tasks.
  • Difficulties with temporal processing: Research published in Clinical Neurophysiology suggests that adults with ADHD may have impaired temporal resolution—the ability to detect rapid changes in sound over time. This can make it harder to decode speech cues, especially in fast-paced or overlapping conversations.

The impact of undiagnosed ADHD on auditory development

When ADHD goes unrecognised during critical developmental periods, individuals may miss out on opportunities to build strong auditory attention and working memory skills—key foundations for effective communication. This can lead to lifelong challenges with:

  • Classroom learning: Where instructions are primarily verbal and attention is expected to be sustained.
  • Social listening: Following group conversations or detecting nuances in tone, irony, or implied meaning.
  • Auditory memory: Recalling details from conversations or verbal instructions, which may be mistakenly attributed to inattention alone.

ADHD is not just a childhood condition

ADHD often persists into adulthood, with around 70–75% of individuals continuing to experience symptoms to some degree. However, adult presentations can look quite different. Some individuals develop effective compensatory strategies or no longer meet full diagnostic criteria, despite ongoing challenges with attention, listening, or cognitive load.

These differences can be subtle. In structured or quiet environments, difficulties may be less noticeable, but they often become more apparent in noisy or complex listening situations where greater auditory attention is required.

Adults with late-diagnosed ADHD frequently describe a lifelong sense of “not hearing properly,” or feeling overwhelmed in background noise. These experiences are often mislabelled as anxiety, hearing loss, or lack of motivation, when they may instead reflect differences in neurological processing.

ADHD or Auditory Processing Disorder?

There is considerable overlap between ADHD and Auditory Processing Disorder (APD), and the two can co-occur. However, they are distinct conditions:

ADHD primarily affects attention regulation across all senses and activities.

APD is specifically about how the brain interprets auditory signals, despite normal hearing.

A comprehensive assessment by both an audiologist (that specialises in APD) and a psychologist or psychiatrist is often necessary to differentiate between them and design effective support strategies.

Supporting adults with ADHD and listening difficulties

For adults with ADHD—diagnosed or suspected— and listening difficulties, there are evidence-based strategies that can help:

  • Hearing support: Even when standard testing suggests hearing is normal, devices like remote microphones or mild-gain hearing aids with advanced noise reduction can reduce listening effort in noisy situations.
  • Environmental modifications: Using quieter meeting spaces, limiting background noise and distractions, and using captions for video content can ease the cognitive load.
  • Voice to text: If you’re attending an important meeting or presentation, utilize dictation note taking tools to catch every word. Many of them now use AI to create comprehensive, structured notes and summaries.
  • Therapeutic support: Auditory training programs and cognitive-behavioural strategies can improve focus and listening endurance.
  • Collaborative care: Working with an audiologist who understands neurodiversity ensures that auditory needs are considered in the context of the whole person.
